This care package provides for the basic needs and support which includes:

companionship and home helper, meal preparation, accompaniment to doctors’ visits (dependent of condition), general housekeeping which could include grocery shopping and fetching of medication, excursions and accompaniment (condition dependent) of loved ones.

This package requires a nurse coordinator to execute an agreed care plan which includes :

Minor step-down care, wound care, mobility care, bathing, bed making, general hygiene care (incontinence care), Assisting with daily living, including encouragement of engagement, mobility and facilitating participation of our clients with minor dementia and alzheimer’s.

This package requires oversight from a professional nurse as well as a higher qualified home-based caregiver.

It requires a well-documented care plan with regular reporting to a doctor and/or a medical facility on the progress of the client. This may also include post-operative care, minor invasive procedures (over seen by a registered nurse) and monitoring (catheter care). Stroke, advanced dementia and alzheimer care, step-down care where the client is not able to move on their own.
